Wolverine got his adamantium back when Apocalypse made him the war horseman. I guess Apocalypse thought Wolverine would be more ” fit ” if he had the adamantium, which doesn’t make a lot of sense because you’d think if Apocalypse thought Wolverine was sub-fit he just wouldn’t have selected him to be a horseman. It’s not ” Only the fit survive except some who can earn a pass with some body mods. ” It’s all glossed over in Wolverine vol. 2 #145, which I admit I haven’t read since it came out.
“Phage” might have been meant to be a reference to microphage, which is a kind/stage of white blood cell.
I’m surprised no one has handled this correctly yet, but a phage is a real thing. It’s a shortening of “bacteriophage”, which is a class of viruses that infect bacteria. And although it’s technically an abbreviation, I’d say it’s more common to hear “phage” in biology than it is to hear “bacteriophage”, much in the same way it’s more common to hear “gas” instead of “gasoline”.
